I fiddled around a bit last night and I think I have ToB access figured out, mainly going by Vhon's, Pugger's, and Alla's posts combined. Here is hopefully a clearer access path.

There are basically 3 sets of quests you need to do. They can thought of as the Arcstone series, the Razorthorn series, and the Takish series. This is a revised edition of Pugger's post.

Mission Skylance 1 : The Library
Mission Skylance 2 : The Obuliette
Mission Skylance 3 : The Laboratory
Recieve Tarnished Chime

Give Tarnished Chime to Ao
Recieve Harmonic Chime
Give Harmonic Chime to Arch Mage Galsin(near arcstone port in)
Recieve Silent Harmonic Chime

(Razorthorn)
Mission 1: Saga Skins
- loot any skin from a red con in Devastation to start and then get the other 8.
- turn in skins to Grand Librarian Maelin in PoK
Recieve the skins back, two quest tomes, and your 70th spell
Give Maelin the two tomes to start:
Mission 2: Becoming the Vessel - complete this for Razorthorn access
Hail Grand Librarian Maelin and say "enraged flesh charm"
Recieve Enraged Flesh Charm
Hail Oathmir the Outcast in Devastation and say "can find" for:
Mission 3: Samples of Corruption
Hail Oathmir after completing the mission and say "what chime"
Give Oathmir your Silent Harmonic Chime
Recieve Twisted Chime

Give Arch Mage Galsin your Twisted Chime
Recieve Twisted Chime (same name but different lore)

You now have access to Theatre of Blood.

It requires you to have someone that has obtained the Enraged Flesh Charm to
request the mission with the trigger text of [can find]. Anyone that have
these charms can request you the
mission for your group if no one has one.

Success you gain AA at general named " Harmonic Dissonance" that transports you to the theater of blood.
